
Autoimmune Gastritis and Vitamin B12 Deficiency: The Overlooked Absorption Problem

Vitamin B12 takes an unusually complicated route from food to bloodstream. First, stomach acid helps release B12 from food proteins. The vitamin then binds to intrinsic factor, a protein made by the stomach's parietal cells. Farther along in the small intestine, this B12-intrinsic factor complex is absorbed.
In autoimmune gastritis, immune activity targets parietal cells and the acid-producing region of the stomach. As parietal cells are lost, stomach acid falls and intrinsic factor production can decline. That creates two obstacles: food-bound B12 is released less efficiently, and less intrinsic factor is available to carry it through the usual absorption pathway.
Pernicious anemia is the late-stage expression most people recognize. It refers to B12 deficiency caused by impaired intrinsic factor activity, often accompanied by enlarged red blood cells and anemia. The American Gastroenterological Association describes pernicious anemia as a late manifestation of autoimmune gastritis and advises clinicians to evaluate both B12 and iron in people with atrophic gastritis.[1]
This distinction matters. A person may have autoimmune gastritis before anemia appears, and a normal-looking blood count does not automatically rule out a developing nutrient problem.
Why Autoimmune Gastritis Vitamin B12 Deficiency Can Be Missed
Autoimmune gastritis rarely announces itself with one unmistakable symptom. Some people have indigestion, early fullness, or upper abdominal discomfort. Others have no digestive symptoms at all. Nutrient changes may be the first visible signal.
A 2024 prospective multicenter study compared people with autoimmune gastritis, people with H. pylori-related gastritis, and controls. Vitamin B12 deficiency occurred in 13.3% of the autoimmune gastritis group, versus 1.5% and 2.8% in the other groups. After adjustment, autoimmune gastritis was associated with substantially higher odds of both B12 and iron deficiency.[2] The study was observational and relatively small, so it cannot predict an individual's outcome, but it reinforces the need to look beyond a single nutrient.

The sequence can also be counterintuitive. Reduced stomach acid may interfere with iron handling before intrinsic factor loss becomes severe enough to produce obvious B12 deficiency. Age can blur the picture further. None of these symptoms is specific to B12 deficiency. Poor sleep, thyroid disorders, iron deficiency, medication effects, and many other conditions can feel similar. That is precisely why symptom-based self-treatment is a poor substitute for testing.
It is also possible to have neurologic symptoms without dramatic anemia. In a 2019 multicenter study of 654 adults with autoimmune atrophic gastritis, subtle blood-count changes and micronutrient deficiencies sometimes appeared before overt anemia.[3] Waiting for hemoglobin to fall can therefore miss an earlier opportunity to investigate.
Timing matters because blood cells and nerves do not always recover on the same schedule. Energy related to anemia may improve as healthy red blood cells return, while numbness or balance changes can take longer. Some longstanding nerve effects may not resolve completely. A careful baseline record of symptoms gives the patient and clinician something concrete to compare with follow-up results, rather than relying on a vague memory of whether things feel better.
Seek prompt medical attention for rapidly worsening weakness, difficulty walking, confusion, chest pain, fainting, or significant shortness of breath. Nerve injury related to prolonged B12 deficiency can become difficult to reverse, making timely assessment important.
The Tests That Clarify the Picture
There is no single universal test panel for every patient. A clinician usually combines symptoms, history, bloodwork, and sometimes endoscopy. The evaluation may include:
- Complete blood count: Hemoglobin, mean corpuscular volume, and red-cell distribution can reveal anemia or changing cell patterns.
- Serum vitamin B12: A useful starting point, although a borderline value may not fully reflect tissue-level status.
- Methylmalonic acid: This often rises when cells do not have enough active B12. Kidney function can affect the result.
- Homocysteine: This can rise with B12 deficiency, but folate status, vitamin B6, kidney function, and other factors also influence it.
- Iron studies and ferritin: Autoimmune gastritis can affect iron as well as B12, and the two deficiencies may alter the expected blood-count pattern.
- Parietal cell and intrinsic factor antibodies: These can support an autoimmune cause. Neither test is perfect, so results need clinical interpretation.
- Gastrin and pepsinogen testing: In selected cases, these markers can offer clues about reduced acid production and stomach-lining atrophy.
If autoimmune gastritis or pernicious anemia is suspected, a gastroenterologist may recommend upper endoscopy with biopsies. This is not simply about confirming the reason for poor absorption. Atrophic gastritis can alter long-term stomach health, and the AGA advises endoscopic risk stratification in people newly diagnosed with pernicious anemia who have not had a recent evaluation.[1]
Testing should also consider other causes of low B12. These include vegan or very low-animal-product diets, bariatric or intestinal surgery, celiac or Crohn's disease, and medicines such as metformin or long-term acid suppressors. More than one factor can be present at the same time.
Why Diet Alone May Not Be Enough
Clams, liver, fish, meat, dairy, and eggs are rich in B12. Fortified foods can help people who avoid animal products. But when intrinsic factor is impaired, simply adding more B12-rich food may not correct the absorption bottleneck.
That does not make a balanced diet irrelevant. Protein, iron, folate, and other micronutrients still contribute to energy and blood health. It means the treatment strategy may need to bypass or overwhelm the usual absorption pathway.
Vitamin B12 can be replaced through injections or high-dose oral therapy. Injections bypass the gastrointestinal tract. High-dose oral B12 relies partly on a small amount of passive absorption that does not require intrinsic factor. A 2024 prospective cohort of people with pernicious anemia found that daily oral cyanocobalamin improved laboratory markers over 12 months.[5] Earlier trials have also suggested that oral and intramuscular routes can both work in selected patients, though regimens, follow-up, and long-term evidence vary.
The right route is individualized. Severity, neurologic symptoms, adherence, access, the cause of deficiency, and the clinician's judgment all matter. Severe or symptomatic deficiency may call for a different plan than a borderline lab value discovered during routine screening.
