Stomach acid and digestive enzymes first release B12 from food. It then binds to intrinsic factor, a protein made in the stomach, and the pair travels to the final part of the small intestine for absorption. Pernicious anemia, gastrointestinal surgery, inflammatory bowel disease, age-related digestive changes, and some medications can interfere with this sequence [1]. The investigators concluded that oral treatment was as effective in that small, short-term study, while clearly calling for longer research [3].
A larger pragmatic trial published in BMJ Open in 2020 randomized 283 adults age 65 and older with B12 deficiency. More than 90 percent in both groups reached the study’s normal B12 threshold after eight weeks. At 52 weeks, 73.6 percent of the oral group and 80.4 percent of the intramuscular group met that threshold. The oral schedule used during maintenance was weekly, while injections were monthly, so schedule design and adherence may matter as much as route [4].
The broadest recent comparison is a 2024 network meta-analysis covering 13 studies and 4,275 patients. Intramuscular administration ranked highest for raising serum B12, but the difference compared with oral treatment was not statistically significant. The researchers also found no significant route differences in hemoglobin, homocysteine, or other measured blood-count outcomes [5].
