
Celiac Disease and Vitamin B12 Deficiency: The Overlooked Link Behind Fatigue and Tingling

Celiac disease and vitamin B12 deficiency can look surprisingly similar from the inside. Both may leave you tired, mentally foggy, short of breath during ordinary activity, or bothered by pins and needles in your hands and feet. When those symptoms linger after going gluten-free, it is tempting to assume the diet is not working. Sometimes the missing piece is not more restriction. It is a nutrient problem that deserves to be measured.
This matters especially in midlife. Celiac disease can be diagnosed at any age, and many adults do not have the dramatic digestive symptoms people expect. They may arrive at a diagnosis through anemia, low bone density, headaches, neuropathy, or fatigue. Vitamin B12 deficiency can be just as quiet. A normal-looking diet, a normal body weight, and even a normal blood count do not always rule it out.

Celiac Disease and Vitamin B12 Deficiency: Why They Can Overlap
Celiac disease is an autoimmune condition in which gluten triggers an immune response that damages the lining of the small intestine. The fingerlike villi that normally create a large surface for nutrient absorption can become shortened or flattened. That can interfere with the absorption of iron, folate, vitamin D, zinc, and other nutrients.
Vitamin B12 has a particularly complicated journey. Stomach acid and digestive enzymes first help release B12 from food. The vitamin then binds to intrinsic factor, a protein made in the stomach. Most absorption happens in the terminal ileum, the last section of the small intestine.
Because celiac injury often appears most prominently in the upper small intestine, B12 deficiency was once considered an unlikely finding. Human studies have challenged that tidy assumption. Untreated inflammation can be extensive, overall digestion may be disrupted, bacterial overgrowth may coexist, and some people have overlapping autoimmune or gastric conditions. A restrictive diet can add another layer if it becomes nutritionally narrow.
In a prospective study of 39 adults with untreated, biopsy-confirmed celiac disease, Dahele and Ghosh found low B12 in 41 percent of participants.[1] A separate series of 159 patients found an overall prevalence of 12 percent, and most cases were not explained by autoimmune gastritis.[2] The exact number varies across studies, but the consistent lesson is that low B12 is plausible enough to check rather than dismiss.

What the Human Research Actually Shows
The research does not support one universal prevalence figure. That is not a flaw so much as a reminder that studies use different B12 cutoffs, populations, diagnostic methods, and stages of treatment.
A Dutch study of 80 newly diagnosed adults found that 87 percent had at least one nutrient value below its reference range. Vitamin B12 was low in 19 percent.[3] In a larger contemporary Mayo Clinic cohort, 5.3 percent of 244 adults with newly diagnosed celiac disease had low B12, compared with 1.8 percent of controls.[4] The absolute figure was smaller, but the difference remained meaningful.
More recent pooled research adds nuance. A 2025 systematic review and meta-analysis of 45 studies found that people with celiac disease clearly had lower levels of several nutrients, but average B12 levels were not significantly different from controls.[5] Another 2025 review reported that people following a gluten-free diet had a lower risk of B12 deficiency than those with untreated celiac disease, although the authors rated the overall evidence quality as very low.[6]
Those findings are not contradictory. An average can look normal even when a clinically important subgroup is deficient. Celiac disease raises the reason to be attentive, but it does not make B12 deficiency inevitable. Individual testing matters more than population averages.
There is also encouraging news. In the Dahele study, B12 levels normalized during follow-up after participants started a gluten-free diet, although symptomatic patients sometimes received supplementation.[1] Healing the intestine can improve absorption. Yet recovery is not immediate, and some adults have another cause of deficiency that remains after gluten is removed.
Symptoms That Deserve a Closer Look
B12 is needed for red blood cell formation, DNA synthesis, and normal nerve function. A shortage can therefore show up in several systems at once. Possible symptoms include:
- persistent fatigue or weakness
- shortness of breath, palpitations, or reduced exercise tolerance
- brain fog, forgetfulness, or difficulty concentrating
- numbness, tingling, burning, or altered sensation in the hands and feet
- balance changes or an unsteady gait
- a sore, smooth tongue or recurring mouth discomfort
- pale skin, headaches, or lightheadedness
- low mood or unusual irritability
None of these symptoms proves B12 deficiency. Iron deficiency, folate deficiency, thyroid disease, sleep problems, medication effects, menopause, depression, and many other conditions can create a similar picture. Celiac disease itself may also cause fatigue while the intestine heals.
Neurologic symptoms deserve prompt attention because they can occur without obvious anemia. Waiting for the red blood cells to become enlarged may delay diagnosis. New weakness, severe balance problems, confusion, chest pain, fainting, or rapidly progressing numbness calls for timely medical evaluation rather than a wellness experiment.
The Tests That Give a Clearer Answer
A serum B12 level is a common starting point, but it is not always the final word. Reference ranges vary, and someone near the lower edge may still need more context. A clinician may consider:
- Complete blood count: This looks for anemia and changes in red blood cell size, while remembering that simultaneous iron deficiency can mask the classic pattern.
- Serum vitamin B12: Useful for screening, but borderline results may require confirmation.
- Methylmalonic acid: This often rises when cells do not have enough usable B12. Kidney function can influence the result.
- Homocysteine: This may increase with B12 deficiency, but folate status, kidney function, thyroid health, and other factors also affect it.
- Ferritin, iron studies, and folate: Celiac-related deficiencies commonly overlap, so testing only B12 can miss part of the story.
- Celiac follow-up markers: Antibody trends, symptoms, diet review, and sometimes repeat endoscopy help assess response to a gluten-free diet.
Timing is important. Ideally, micronutrients are measured around diagnosis and followed after treatment begins. If you have already started B12, tell the clinician the form, dose, route, and timing because supplementation can change laboratory results.
Do not start a gluten-free diet solely to test whether symptoms improve before completing a celiac evaluation. Removing gluten can lower celiac antibodies and allow intestinal healing, making the diagnosis harder to confirm. Work with a gastroenterology professional on testing first.
Why Going Gluten-Free May Not Be the Whole Solution
A strict gluten-free diet is the foundation of celiac treatment. It removes the immune trigger and gives the small intestine an opportunity to heal. It is not, by itself, a guarantee that every nutrient gap will immediately close.
First, healing takes time. Second, gluten-free packaged foods are not automatically nutrient dense. Many are lower in fiber and may not be fortified in the same way as conventional grain products. Third, celiac disease can coexist with conditions that affect B12 independently, including pernicious anemia, autoimmune gastritis, inflammatory bowel disease, or prior gastrointestinal surgery.
Medications matter too. Metformin and long-term acid-suppressing drugs can be relevant to B12 status. Age-related changes in stomach acid can make food-bound B12 harder to access.
