Why Metformin and Vitamin B12 Deficiency Are Connected

Vitamin B12 from food must complete a surprisingly complex journey before the body can use it. Stomach acid helps release B12 from protein. The vitamin then binds to intrinsic factor, a protein produced in the stomach, and that complex is absorbed near the end of the small intestine.

Metformin appears to interfere with this process. The exact mechanism is still being studied, but the leading explanation involves calcium-dependent absorption of the intrinsic factor and B12 complex in the ileum. Metformin may disrupt that step, reducing how much B12 enters circulation over time.[1]

This is generally a gradual effect, not something that happens after a few doses. Risk tends to rise with longer treatment and higher daily doses. A landmark randomized, placebo-controlled trial followed 390 people with type 2 diabetes for about 4.3 years. Compared with placebo, metformin was associated with a 19% average decrease in B12 concentration. The absolute risk of deficiency was 7.2 percentage points higher in the metformin group.[2]

Longer follow-up from the Diabetes Prevention Program Outcomes Study reached a similar conclusion. Combined low and borderline-low B12 was more common among metformin users at both five and 13 years. Each additional year of metformin use was associated with higher odds of deficiency.[3]

None of this means metformin is unsafe or inappropriate. It means B12 status is one more useful data point to monitor while taking it.

Why the Risk Can Matter More After 40

Age 40 is not a biological cliff, but several factors can begin to stack up in midlife. Stomach acid production may decline with age. Acid-suppressing drugs such as proton pump inhibitors can further reduce the release of food-bound B12. Vegetarian and vegan diets contain little or no natural B12 unless fortified foods or supplements are included. Prior bariatric or gastrointestinal surgery can also affect absorption.

Metformin may add to these existing pressures. A 2024 systematic review of 21 studies found a significant association between metformin use and lower B12 levels in 17 studies. Higher dose and longer duration emerged as the most consistent risk factors.[4]

A separate 2024 human study illustrates how these factors may combine. Among adults with type 2 diabetes taking metformin, 18.6% had B12 deficiency. After adjustment, a metformin dose above the study median was associated with substantially higher odds of deficiency. People taking at least 1,000 mg daily for 48 months or longer were identified as a higher-risk group.[5]

Your personal risk may be higher if you:

  • Have taken metformin for several years
  • Take a higher metformin dose
  • Are over 50
  • Eat little or no animal-derived food
  • Regularly use an acid-suppressing medication
  • Have inflammatory bowel disease, celiac disease, or prior stomach or intestinal surgery
  • Already have anemia or neuropathy symptoms

Vitamin B12 deficiency does not always produce obvious symptoms. Some people have low blood levels before they notice anything at all. When symptoms occur, they may include:

  • Shortness of breath with routine activity
  • Pale skin or a sore, smooth tongue
  • Memory difficulty, low mood, or mental fog
  • Tingling, numbness, or burning in the hands and feet

The nerve-related signs deserve special care. Diabetes itself can cause peripheral neuropathy, so tingling or numbness may be attributed to blood sugar without evaluating B12. Conversely, not every case of neuropathy in a metformin user is caused by B12 deficiency.

A 2022 review concluded that metformin-associated B12 deficiency may cause or worsen several forms of neuropathy, while also emphasizing the need to identify and treat the underlying deficiency.[6] Evidence is stronger for the relationship between metformin and lower B12 than it is for proving that B12 explains every neurological symptom.

Seek prompt medical evaluation for new weakness, trouble walking, loss of bladder or bowel control, one-sided symptoms, chest pain, or severe shortness of breath. Those are not symptoms to manage through wellness care alone.

How to Test Vitamin B12 Status

A serum vitamin B12 test is usually the starting point. The result is useful, but it is not always the complete answer. Laboratory reference ranges vary, and a borderline value may not show how much functional B12 is available inside tissues.

  • Methylmalonic acid (MMA): This often rises when cells do not have enough usable B12. Kidney function can affect the result.

  • Complete blood count: This looks for anemia and enlarged red blood cells, although neurological B12 deficiency can occur without anemia.
  • Folate and iron studies: These can help distinguish overlapping causes of fatigue or abnormal blood counts.

Testing is especially worth discussing if you have anemia, new or worsening neuropathy, a restrictive diet, gastrointestinal disease, or years of metformin exposure. Clinical guidance commonly recommends periodic B12 assessment in metformin-treated patients, particularly when anemia or peripheral neuropathy is present.[3]

There is no single screening interval that fits everyone. Your clinician may test sooner or more often based on dose, duration, symptoms, diet, other medications, and previous results. Do not stop metformin while waiting for a test unless the prescriber specifically tells you to do so.

Treatment Options: Food, Oral Supplements, and Injections

Once deficiency is confirmed, the best form of B12 support depends on its severity, symptoms, and likely cause.

Intramuscular or subcutaneous B12 may be considered when deficiency is significant, neurological symptoms are present, absorption is unreliable, or a clinician wants a more direct replacement route. A systematic review of seven clinical trials in metformin-treated adults found that B12 supplementation consistently increased serum B12. Effects on neuropathy symptoms were less certain, with mixed results across studies.[7]

This distinction matters. Raising a low laboratory value is not the same as guaranteeing more energy or reversing nerve symptoms. Earlier evaluation is valuable because prolonged neurological injury can be harder to reverse.
