
NAD+ Injection Side Effects: Safety, Symptoms, and What to Expect

If you are exploring NAD+ therapy for energy, focus, or healthy aging, the benefits are probably only half of your research. NAD+ injection side effects deserve the same attention. Online descriptions can make this therapy sound either completely effortless or unusually intense. The more useful answer sits in the middle: reactions depend on the route, dose, formulation, pace of administration, and your individual health profile.
Human research on direct injectable NAD+ is still limited. Early studies offer useful safety clues, but they do not establish long-term safety or predict exactly how every patient will feel. That makes physician screening, a reputable compounding pharmacy, clear dosing instructions, and thoughtful follow-up more than administrative details. They are part of the treatment itself.
Here is a practical, evidence-aware guide to what researchers have observed, what remains uncertain, and when a symptom needs medical attention.
What NAD+ is and why delivery method matters
Nicotinamide adenine dinucleotide, shortened to NAD+, is a coenzyme found in every living cell. It helps enzymes transfer energy from food into usable cellular energy. It also participates in DNA repair, stress-response signaling, and the activity of proteins associated with cellular maintenance.
NAD+ biology has become a major longevity research topic because tissue levels and NAD-related metabolism can change with age. That does not mean replacing NAD+ has been proven to slow aging or treat a disease. It means researchers have a plausible biological pathway to study.
The body can approach that pathway in several ways. Direct NAD+ may be administered intravenously or by an injection under the skin. Oral products usually contain precursors such as nicotinamide riboside, or NR, and nicotinamide mononucleotide, or NMN, which the body uses to build NAD+.
Those routes are not interchangeable. An IV infusion enters the bloodstream over a controlled period. A subcutaneous injection deposits a smaller volume into tissue for absorption. An oral precursor passes through the digestive system and is metabolized before contributing to the NAD pool. A reassuring oral NR trial cannot, by itself, prove that injected NAD+ has the same side-effect profile.
That distinction is easy to lose in wellness headlines, but it is central to making sense of the evidence.
NAD+ injection side effects in human studies
The most directly relevant published evidence is small. In a 2019 pilot study, Grant and colleagues gave eight healthy men 750 mg of NAD+ through a six-hour IV infusion, while three participants received saline. The researchers tracked NAD+ and its metabolites in blood and urine. They found rapid metabolism during the early infusion period and reported no adverse events in either group.[1]
“No adverse events were observed during the 6 h infusion.”[1]
That finding is encouraging, but the study included only 11 men, tested one IV protocol, and was designed mainly to study metabolism rather than detect uncommon side effects. It cannot answer questions about women, people with chronic conditions, subcutaneous injections, repeated long-term use, or different doses.
A 2026 retrospective pilot study provides a less comfortable but equally important view. Reyna and colleagues reviewed real-world IV treatment in a very small group. All six people receiving NAD+ IV reported temporary moderate-to-severe symptoms during infusion, including abdominal cramping, nausea, diarrhea or vomiting, increased heart rate, throat discomfort, congestion, or chest pressure. Symptoms stopped when the infusion ended, and follow-up laboratory markers did not show clinically significant changes in liver, kidney, inflammatory, cardiovascular, or thyroid measures.[2]
Why did the two direct-NAD+ studies look different? The protocols, infusion rates, settings, participant selection, symptom collection, and study designs differed. With samples this small, even a few people can change the overall picture dramatically. Together, the studies suggest two responsible conclusions: short-term treatment has not produced a clear organ-safety signal in these limited reports, and acute tolerability symptoms can still be significant.
Research on oral NAD+ precursors adds context, not proof. In a randomized crossover trial of 30 healthy adults ages 55 to 79, six weeks of oral NR at 1,000 mg daily increased blood NAD+ and was generally well tolerated. Reported symptoms during NR included nausea, flushing, leg cramps, and increased bruising, while other symptoms also occurred during placebo.[3] Another randomized trial in 140 healthy overweight adults found eight weeks of NR at 100, 300, or 1,000 mg daily was well tolerated without clinically meaningful changes in safety laboratory results.[4]
A 10-week trial in 20 older adults with mild cognitive impairment likewise found no difference in adverse-event reporting between oral NR and placebo, although the sample was too small to establish broad safety.[5] These studies help explain the growing medical interest in NAD metabolism. They should not be read as direct safety studies of NAD+ injections.
Common symptoms and what they may feel like
Published IV data and clinical experience point to a cluster of short-lived symptoms. The exact frequency after subcutaneous NAD+ injections has not been established in large peer-reviewed trials.
Possible local reactions include:
- Tenderness, stinging, redness, itching, or mild swelling at the injection site
- A small temporary lump where fluid was deposited
- Bruising, particularly in people who bruise easily or take medicines that affect clotting
Possible whole-body symptoms may include:
- Nausea or stomach discomfort
- Headache, lightheadedness, or a flushed feeling
- Fatigue or a generally unwell sensation
- Muscle tension or cramping
- A temporary awareness of a faster heartbeat
Most mild local reactions should improve rather than intensify. Rotating approved injection sites and following the supplied storage, preparation, and administration instructions can help reduce avoidable irritation. Do not change the prescribed dose, concentration, or schedule on your own in an attempt to “push through” a reaction.
Some symptoms should never be normalized as part of wellness therapy. Seek urgent medical help for chest pain or pressure, trouble breathing, fainting, facial or throat swelling, widespread hives, confusion, severe weakness, or a sustained rapid or irregular heartbeat. Increasing warmth, spreading redness, pus, fever, or escalating pain at an injection site may indicate infection and requires prompt assessment.

Side effects are rarely about the ingredient alone. Concentration, injection volume, route, dosing frequency, hydration, concurrent medications, and individual sensitivity can all shape tolerability.

Long-term uncertainty deserves special emphasis. The direct human NAD+ studies are too small and brief to determine whether uncommon or delayed risks exist. Oral NR studies have generally been short, often lasting weeks rather than years. There is also no robust evidence that more NAD+ produces greater wellness benefits. A conservative, individualized approach is more scientifically defensible than escalating simply because a dose is popular online.

A clinician should review your goals and ask whether symptoms such as fatigue, poor concentration, or low exercise tolerance could have another explanation. A practical checklist before your first dose
