
Vitamin B12 and Restless Legs: Could a Deficiency Be Disrupting Your Sleep?

Vitamin B12 and restless legs can become an unexpectedly urgent question at 11 p.m. You are tired, the room is quiet, and yet your legs seem determined to keep moving. There may be crawling, pulling, tingling, or a deep internal restlessness that improves only when you stand up and walk. Then it returns as soon as you settle back into bed.
Restless legs syndrome has several possible contributors, and vitamin B12 is only one of them. Iron status, medications, pregnancy, kidney disease, nerve health, sleep apnea, and family history can all matter. Still, because B12 helps maintain nerves and red blood cells, checking it may be reasonable when nighttime symptoms arrive alongside fatigue, numbness, balance changes, dietary restriction, or medications that affect absorption.
The evidence is nuanced. One human study found markedly lower B12 levels in people with restless legs syndrome, while a much larger meta-analysis did not find a consistent difference overall.[1][2] That tension is exactly why a thoughtful evaluation matters more than guessing or reaching for a one-size-fits-all supplement.
What restless legs syndrome actually feels like
Restless legs syndrome, often shortened to RLS, is a neurological sensorimotor condition. Its defining feature is an urge to move the legs, usually accompanied by uncomfortable sensations. Symptoms begin or worsen during rest, improve temporarily with movement, and tend to be more noticeable in the evening or at night.
People describe it in very different language: fizzing, buzzing, pulling, creeping, aching, electric, itchy deep inside, or simply impossible to ignore. The sensation is not always painful. Some people mainly experience an internal drive to stretch or pace. Because movement brings short-term relief, repeated sleep disruption can follow.
RLS is not the same as a nighttime muscle cramp. A cramp typically involves a sudden, painful muscle contraction that you can feel or see. It is also different from the numbness and burning of peripheral neuropathy, although symptoms can overlap and both conditions can exist together. Periodic limb movements during sleep are repetitive movements observed while someone is asleep. They can accompany RLS, but they are not the same diagnosis.
The pattern matters. A clinician will want to know when symptoms occur, what relieves them, whether they affect one or both legs, and whether there is persistent weakness, swelling, skin color change, or pain. New one-sided swelling, chest pain, sudden weakness, or loss of bladder or bowel control warrants urgent medical attention rather than a wellness approach.
Vitamin B12 and restless legs: what the research shows
A 2022 case-control study compared 80 adults with RLS with 80 age- and sex-matched controls. The RLS group had significantly lower serum B12, and lower B12 was independently associated with the presence of RLS. B12 levels also tracked inversely with symptom severity in the study.[1] This was an intriguing signal, but an observational design cannot prove that low B12 caused the symptoms.

That caution proved important. In 2025, researchers pooled evidence from 21 studies covering 1,298 people with RLS and 3,629 controls. They found no statistically significant overall difference in B12 levels and no clear difference in the odds of B12 deficiency.[2] The studies varied substantially, which makes a universal conclusion difficult, but the meta-analysis does not support treating B12 as a general remedy for every case of restless legs.
There are also no controlled trials showing that B12 supplementation treats RLS itself.[2] That is a key distinction. Correcting a confirmed deficiency is sound clinical care. Taking B12 in the hope that it will quiet restless legs when B12 status is already adequate is not supported by direct evidence.
The practical takeaway is balanced: B12 deficiency can affect nerves and may imitate or intensify sensory complaints, so it deserves consideration in the right person. It should be evaluated as one possible contributor, not presented as the default explanation.
Why B12 status may still matter to your nerves
Vitamin B12 is required for normal DNA synthesis, red blood cell formation, and neurological function. Inadequate B12 can disrupt myelin, the protective material surrounding nerve fibers, and may contribute to numbness, tingling, weakness, reduced reflexes, or unsteady walking.
A systematic review and meta-analysis found that peripheral neuropathy was associated with lower plasma B12 and higher levels of methylmalonic acid and homocysteine.[3] Those findings do not turn peripheral neuropathy into RLS, but they explain why clinicians may investigate B12 when leg sensations do not fit neatly into one category.
Deficiency becomes more plausible when restless sensations appear with:
- persistent numbness or pins and needles in the hands or feet
- unusual fatigue, weakness, or shortness of breath
- a smooth, sore, or burning tongue
- balance changes or a less steady gait
- memory, concentration, or mood changes
- a vegan or very low animal-food diet without reliable B12 intake
- long-term use of metformin or acid-suppressing medication
- a history of bariatric surgery, inflammatory bowel disease, or pernicious anemia
Neurological symptoms can occur without dramatic anemia. That is one reason a normal-looking blood count should not automatically end the discussion when symptoms and risk factors point toward B12 deficiency.
The tests that provide useful context
Serum B12 is usually the starting point, but it is not a perfect stand-alone test. Results near the low end of a laboratory range can be difficult to interpret, especially when symptoms are present. Recent biomarker reviews recommend combining a measure of circulating B12 with a metabolic marker when deficiency remains possible.[4]
Methylmalonic acid, or MMA, rises when cells do not have enough usable B12. Homocysteine may also rise, although it is influenced by folate, kidney function, thyroid status, and other factors. Holotranscobalamin, sometimes called active B12, is another option in certain settings. Kidney function matters because reduced filtration can elevate MMA independently of B12 status.
For restless legs specifically, iron testing takes priority. The American Academy of Sleep Medicine recommends regular iron studies in clinically significant RLS, including ferritin and transferrin saturation.[5] A person can have a blood count that does not show anemia while still having iron stores that are relevant to RLS. Iron should not be taken blindly, since excess iron can be harmful.
A reasonable clinician-guided workup may include:
- complete blood count
- serum B12, with MMA or homocysteine when appropriate
- ferritin, serum iron, total iron-binding capacity, and transferrin saturation
- folate, kidney function, glucose or A1C, and thyroid testing based on history
- review of medications, alcohol and caffeine intake, pregnancy status, and sleep symptoms
Testing should answer a clinical question. It is not about chasing an “optimal” number in isolation. The goal is to connect symptoms, risk factors, examination findings, and laboratory results.
Look beyond vitamins before blaming your legs
Modern RLS guidance starts by addressing factors that may worsen symptoms. Caffeine and alcohol can be triggers for some people. Sedating antihistamines, certain antidepressants, and dopamine-blocking medicines may aggravate RLS, although prescribed medication should never be stopped without discussing it with the prescriber.[5]
Untreated obstructive sleep apnea can fragment sleep and complicate the picture. Kidney disease, pregnancy, iron deficiency, and some neurological conditions may also be associated with RLS. Family history is common in primary RLS.
For a week or two, a simple symptom diary can make a medical appointment more productive. Record when the sensations begin, how long they last, what you ate or drank, medications taken, exercise timing, menstrual blood loss if relevant, and what brought relief. Note whether symptoms are truly worse at rest and at night. That characteristic pattern helps separate RLS from cramps, joint pain, vascular symptoms, and neuropathy.
Gentle movement, stretching, massage, a warm or cool bath, consistent sleep timing, and moderate daytime exercise may offer temporary relief. Very intense exercise close to bedtime can worsen symptoms for some people. These strategies may support comfort, but persistent sleep disruption still deserves evaluation.
When B12 treatment may be appropriate
If testing confirms B12 deficiency, treatment should be matched to its cause and severity. Food sources include meat, fish, eggs, and dairy, while fortified foods can help people eating plant-based diets. Oral B12 is effective for many people, but absorption problems, significant neurological symptoms, adherence challenges, or clinician judgment may make an injectable route appropriate.
Evidence from peripheral neuropathy research helps set expectations.
