Vitamin B12 deficiency from metformin can be easy to miss. Fatigue gets blamed on a demanding week. Tingling feet may be written off as circulation or diabetes. A fuzzy sense of concentration can feel like an ordinary part of midlife. Yet for people who take metformin, especially at a higher dose or for several years, low B12 deserves a place on the health checklist.

That does not make metformin a bad medication. It has decades of clinical use and remains important for many people with type 2 diabetes, prediabetes, or polycystic ovary syndrome. The useful question is not whether to fear it, but how to monitor a known nutritional effect without abandoning a treatment that may be helping.

Here is what the research says about risk, symptoms, testing, and the point at which a clinician may recommend oral or injectable vitamin B12.

Why metformin can lower vitamin B12

Vitamin B12, also called cobalamin, helps the body make red blood cells and DNA. It is also needed for healthy nerve function. We obtain it mainly from animal foods and fortified products, then absorb it through a multistep process involving stomach acid, intrinsic factor, calcium, and receptors in the final part of the small intestine.

Metformin appears to interfere with this absorption process. The exact mechanism has not been fully settled, but a leading explanation involves disruption of the calcium-dependent binding needed for the intrinsic factor-B12 complex to be taken up in the ileum.[1] Changes in intestinal movement, bacterial balance, and bile acid metabolism have also been proposed.

The effect is usually gradual because the liver stores a substantial reserve of B12. Someone can eat a reasonably balanced diet and still see levels drift downward over years. That slow timeline is one reason symptoms and medication history may not immediately be connected.

Evidence is stronger than a simple association. In a randomized, placebo-controlled trial published in BMJ, 390 people receiving insulin for type 2 diabetes were assigned to metformin or placebo for about 4.3 years. The metformin group experienced an average 19% decrease in B12 concentration. The absolute risk of biochemical B12 deficiency was 7.2 percentage points higher than with placebo.[2]

That is a short conclusion from the investigators, not a reason to stop metformin independently. Medication changes should always be discussed with the prescribing clinician.

How common is vitamin B12 deficiency from metformin?

There is no single prevalence figure that applies to everyone. Studies use different cutoffs for deficiency, include different populations, and measure exposure over different periods. Still, a consistent pattern appears across trials and observational data: metformin users tend to have lower B12 levels, and risk generally rises with dose and duration.

A 2014 systematic review of six randomized trials found that B12 concentrations were, on average, 53.93 pmol/L lower with metformin than with placebo or another treatment. The difference was larger in trials using at least 2,000 mg per day, supporting a dose-response relationship.[3]

Long-term data from the Diabetes Prevention Program Outcomes Study add another useful perspective. At year five, low B12 was found in 4.3% of participants assigned to metformin and 2.3% assigned to placebo. When low and borderline-low results were combined, the difference was 19.1% versus 9.5%. Each additional year of metformin exposure was associated with a 13% increase in the odds of deficiency.[4]

A 2024 review examined 21 peer-reviewed studies published since 2015. Seventeen found a significant relationship between metformin use and B12 levels. Higher doses, longer treatment, older age, and some dietary or demographic factors appeared to shape individual risk.[5]

Risk may be more relevant if you:

  • Have taken metformin for four or more years
  • Take more than 1,500 mg per day
  • Eat a vegan or low-animal-product diet
  • Use acid-suppressing medications for a long period
  • Have had bariatric or gastrointestinal surgery
  • Have inflammatory bowel disease, celiac disease, or another absorption disorder
  • Are older, since stomach acid and absorption can decline with age
  • Already have anemia, numbness, tingling, or balance changes

These are prompts for a conversation and testing, not a diagnostic checklist.

Symptoms that deserve attention

Low B12 can affect blood formation, nerves, mood, and cognition. Symptoms vary, and some people have an abnormal lab result before noticing anything. Others develop neurological symptoms without obvious anemia.

Possible signs include:

  • Persistent fatigue, weakness, or reduced exercise tolerance
  • Shortness of breath, dizziness, or a fast heartbeat
  • Pale skin or a sore, smooth tongue
  • Numbness, pins and needles, or burning sensations in the hands or feet
  • Poor balance or an unsteady gait
  • Trouble concentrating, memory changes, or low mood
  • Reduced appetite or unexplained weight loss

There is an important overlap here. Diabetes itself can cause peripheral neuropathy, while anemia, thyroid disorders, poor sleep, depression, and other nutrient deficiencies can also cause fatigue or brain fog. Symptoms alone cannot identify B12 deficiency.

The overlap with diabetic neuropathy makes timely assessment particularly valuable. If a person has tingling feet and metformin exposure, assuming every sensation is simply “the diabetes” could delay treatment of a reversible contributor. Prolonged severe B12 deficiency can cause lasting nerve injury, so new weakness, trouble walking, rapidly worsening numbness, confusion, or vision changes warrants prompt medical evaluation.

The right tests go beyond a casual B12 check

Serum vitamin B12 is the usual starting point, but interpretation is not always black and white. Laboratory reference ranges vary, and a borderline result may not reveal whether cells are receiving enough usable B12.

A clinician may consider:

  • Complete blood count: Looks for anemia and enlarged red blood cells, although normal results do not exclude neurological B12 deficiency.
  • Serum vitamin B12: A practical first-line measure, interpreted alongside symptoms and risk factors.
  • Methylmalonic acid (MMA): Often rises when cellular B12 is inadequate and can help clarify a borderline result. Kidney dysfunction can also raise MMA.
  • Homocysteine: May rise with B12 deficiency, but it is less specific because folate, vitamin B6, kidney function, and other factors influence it.
  • Folate, iron, thyroid, or metabolic testing: May be useful when fatigue, anemia, or neurological symptoms have more than one possible cause.

Testing frequency should be personalized. High-dose oral B12 works for many people because a small percentage can be absorbed passively, even when the usual intrinsic-factor pathway is impaired. A 2021 randomized, double-blind trial followed 90 metformin-treated adults with diabetic neuropathy and B12 levels below 400 pmol/L. Daily oral methylcobalamin for one year raised B12 and improved several nerve-function, pain, and quality-of-life measures compared with placebo.[6] This was a specific population and does not prove that B12 will improve neuropathy in everyone.
